- Fax the prescription / referral notes to (250) 828-7824 (preferred), or call (250) 828-7886.
- Include patient demographics, diagnosis, device or goal requested, relevant precautions, and funding context when known (WorkSafeBC claim #, ICBC, PharmaCare, extended health, DVA/NIHB, etc.).
- We assist with prior authorization and claim forms where applicable. Patients can also review our first visit guide.
